A clinical examination with Dr. White may follow, or our team will work with you to schedule a follow-up appointment. Whatever the case is, patients can rest assured that they’ll never be left alone – you’ll be escorted everywhere and made to feel right at home with each member of our staff.

Now, our patients can download the paperwork they’ll need through the links below for additional convenience! Simply fill in your information digitally, print, sign, and bring each form with you. Alternatively, you may print the blank form, write in your information, and sign using a black ink pen only, please. Bring each completed form with you for our front desk team when you arrive for your first visit.
